Parental Consent Form

This form is to inform the parents and/or legal guardians of any player or potential player under the age of 18 years old of what type of game From The Ashes is.

Vampire 101
The game that we run/play is done in the World of Darkness, a roleplaying system made by White Wolf Inc.
Vampire is an adult natured game where players get to play as a vampire or a vampire's servant (ghoul).
This game has a high level of intrigue and can contain scenes that may not be suitable for minors. More information on "Vampire The Masquerade" can be found by searching on the internet, or at http://white-wolf.com

LARP 101
Live Action Roleplay (LARP) is a version of some games that are acted out, a lot like a theatre group.
LARP does have modified rules to simplify roleplay and storytelling. Of note LARP has a few base rules that bear mentioning for saftey sake. These rules are printed in the front of any of the main LARP books published by White Wolf, and are echoed in our House Rules Disciplinary Section, which can be found at the bottom of this link: http://ftalarp.com/index.php/HouseRules

All juviniles (those under the age of 18) are required to have a copy of one of these documents signed and dated by their parent or legal guardian before being allowed to attend or play at From The Ashes events. Once we have a copy of this document on file, no further action is required on behalf of the parent or legal guardian. In the event that the parent or legal guardian later decides to not allow the signed for individual to attend From The Ashes events, the parent or legal guardian must notify the Staff of From The Ashes by one of the following means:
